How to enroll in BENLYSTA Gateway

  1. Download the enrollment form
    BENLYSTA Gateway Enrollment Form
  2. Complete and submit the form
    You’ll need to work with your doctor to fill out the form and submit it to BENLYSTA Gateway.
  3. BENLYSTA Gateway will contact you
    After contacting your insurance and determining coverage options, BENLYSTA Gateway will contact you within 1 week.
  4. Get approval
    Your doctor will handle your prescription once approved. Call your doctor’s office to check the status of your prescription if you don’t hear back within 2 weeks of the first call.

    • Patient has a commercial medical or prescription insurance plan

    AND

    • Patient is a resident of the US (including the District of Columbia, Puerto Rico, and the US Virgin Islands); and
    • Not eligible for or enrolled in a government-funded program that provides prescription drug coverage*

    If you don’t hear from BENLYSTA Gateway, your doctor’s office, or specialty pharmacy in the 2 weeks after you send in your form, call your doctor’s office to inquire about it or contact the BENLYSTA Cares Support Team at 1-877-4-BENLYSTA (1-877-423-6597) to check on the progress. Once Gateway has the information they need, they can prepare a summary of benefits typically within 1 week.
